Family Support Services
The Family Service Department is comprised of Social Workers,
Early Intervention Service Coordinators and OMRDD Medicaid Service Coordinators. Our primary
goal is to empower the parents to be their child's own best
advocate.
Social Workers provide individualized support and services for
families of children birth to five as identified by the child's IFSP
(birth - 3) or IEP (3 - 5).
Medicaid Service Coordinators provide consumers of all ages with
individualized support and services based on their specific needs or
personal valued outcomes as identified by their ISP.
Services can include:
- Provide support for families regarding their child's
disability
- Crisis intevention
- Community referral and linkage to services
- Family empowerment
- Advocacy
- Parent education
- Information and resources
- (OMRDD) Medicaid service coordination
- (OMRDD) Family education and training
- (OMRDD) HCBS Plan of Care Support Service (POCSS)
- Early intervention service coordination
- Early intervention social work services
- Family training (CPSE)
- Parent support group facilitation
- CPSE diagnostic evaluating team responsible for social
histories
- CPSE representation for diagnostic evaluations
- Assistance with issues related to abuse/neglect
